Patients with Asthma, COPD, Diabetes, Non-Diabetic Hyperglycaemia (NDH), Learning Disabilities and Rheumatoid Arthritis will require annual reviews which will be performed on the quarter of their birthday. eg patient with a birthday in June will be asked to attend between April-June and Patient with December birthday will be called between October – December
